Dr Hart's patients: Dear Patients,

The day has arrived that my family medicine practice is now closed.

After careful consideration, I will be changing my scope of practice to provide care in urgent care and to expand my women’s health and menopause clinic.

Unfortunately I will not be able to take any of my current family medicine patients with me, and women's health patients  will have to be referred back to me by their new primary care provider.

I will also continue my Migraine Botox clinic. Patients currently receiving Botox treatments will be contacted with information about continuing their care at my new office.

• Ask your specialist(s) if they can assist with interim care

Dr. Arjang and Dr. Fatoorechi will continue their practices at Innovations medical. 

If you have:

• Standing orders under my name - Please destroy them and get a new standing order from your new doctor.
• Booked imaging (e.g., MRI, CT) or specialist visits → these may proceed as planned

➡️ Please ensure your new provider’s name is updated with those services so results are directed appropriately.

If you are receiving controlled medications and are having difficulty finding a new prescriber, you may contact:

After the clinic closes:

• PCN services like primary care nursing - Please connect to your new doctor's PCN team for nursing and mental health support

Due to the size of most patients charts we are unable to fax more than a summary to your new doctor. Instead we can copy your file to a flash drive/memory stick. We ask that you come in to sign a release form and the front desk staff will prepare it for you, for pick up. 

Due to the hardware and time involved we ask $25 per flash drive. 

• Dr Arjang and Dr Fatoorechi's patients charts will be transferred electronically to Innovation medical, and no further action need to be taken by their patients.
• Patients of dr. Hart who transfer to other doctors at Innovations West or South will also be sent electronically during the chart transfer. 

• A copy of your medical records will also be securely retained in accordance with Alberta’s Health Information Act.

• After clinic closure:
• Most key health information (lab results, imaging, specialist reports) is available to healthcare providers via:
• Alberta Netcare
• Connect Care
• You may also access parts of your health information through:
• MyHealth Records

• Information about how to request your records will be available through the
  College of Physicians and Surgeons of Alberta website https://CPSA.ca
• A reasonable administrative fee may apply, as permitted by legislation

• Clinic phone and  email has now been turned off.

• Please download or save any needed information from the AVA portal before August 27, 2026

How should I book my appointments?

• For appointments after August 17, 2026:  Any appointments already booked will be automatically transferred to our new system.

I accept referrals for IUD and Nexplanon insertions for people looking for long-term contraception options. Endometrial biopsies as referred by your Family Doctor and igraine Botox. All doctors will continue to follow up with our current referred patients at our new locations.
